研究目的:觀察針灸緩解頸部疼痛之立即效果。
重要性:利用三維頸椎活動測量儀觀察患者針灸後活動角度的改變,及疼痛量測計客觀評估針灸止痛情形。
材料與方法:本研究採單盲隨機分配設計,受試者為介於20~65歲、頸部疼痛期間小於6個月患者,計32人分為兩組。選用穴位包括:曲池、肩井、肩中兪、肩外兪、陽陵泉及完骨。針具採用Park偽針器。針刺穴位最少2個,最多6個,留針20分鐘,右(左)側痛針右(左)側,若雙側痛,則僅針刺較痛一側,針灸治療共1次。評估的工具為視覺疼痛量表、疼痛量測計、三維頸椎活動測量儀。評估時間為治療前、治療後及治療後1小時。
結果:組內比較,以Repeated measurement ANOVA,視覺疼痛量表:兩組都有顯著差異(p<0.05)。疼痛量測計:針灸組在最大疼痛忍受度,治療前與治療後1小時有顯著差異;偽針組在起始感覺及最大疼痛忍受度幾乎都有顯著差異。頸部活動角度:針灸組除前彎角度外,都有顯著差異;偽針組僅在患側旋轉角度有顯著差異。組間比較,則用Independent t test,治療後1小時在患側旋轉角度有顯著差異。
以上結果顯示,針灸治療可以降低患者疼痛的感覺,有止痛效果,並且可以改善患者頸部活動的角度。
Background: Apply Cervical Range of Motion Instrument & Algometer to measure effects of neck pain after acupuncture therapy.
Purpose: Our aim is observation on therapeutic therapeutic effects of acupuncture on neck pain.
Methodology: The study was a single-blind randomized controlled trial. All the participants were neck pain of less than 6 months and ages between 20 to 65 years old. Thirty two outpatients were divided into 2 groups. Park Sham Device was adopted in the research. Participants were inserted needles unilateral acupoints between 2 to 6 acupoints on lesion side and retained 20 minutes. Participants received 1 times treatment totally. The measurement tools were contained VAS, Algometer and Cervical Range of Motion Instrument and measured at baseline, after treatment and 1 hour follow up.
Outcomes: Compared with the therapeutic effects of internal group by repeated measurement ANOVA. VAS: Two groups had significant difference(p<0.05). Algometer: Before treatment and 1 hour follow up had significant difference on intolerance pain of real acupuncture; Initial feeling and intolerance pain of sham acupuncture hardly had significant difference. CROM: Range of motion had significant difference of real acupuncture except flexion; On sham acupuncture lesion side rotation had significant difference only. The therapeutic effects of two groups compared with independent t test. It showed significant difference only on 1 hour follow up.
Conclusion: It display pain feeling was decreasing, pain threshold and cervical range of motion was increasing after acupuncture.
