    题名: 一氧化氮在末稍神經電刺激鎮痛所扮演的角色---福馬林試驗
    The Role of Nitric Oxide in Analgesia of Eelectric Stimulation of Peripheral Nerve on Formalin-Treated Rats

    摘要: 我們先前的研究已知電刺激末稍神經能減少Formalin 所誘發的疼痛,但鎮痛機轉至今仍然不清。一氧化氮(NO)是一種神經傳遞物質,它在維持人體正常生理功能上扮演一個重要的角色。NO 在脊髓內能調控嗎啡的止痛作用,在癲癇或腦缺血的實驗模式中,電針會減少NO 的產生,由此可知NO 和電針作用之間有密切的關連。因此本研究的目的探討NO與電刺激末稍神經的止痛的關係。我們將48隻雄性Sprague-Dawley(SD大鼠分成八組,每組六隻。第一組於大白鼠之左後肢Plantar表面皮下注射50 ul之Normal saline,第二組注射等量之福馬林(5%,50 ul),第三組在注射福馬林之前、後15分鐘分別以2Hz電刺激左側坐骨神經處理15分鐘,第四組在電刺激前15 分鐘先注射NOS 之抑制劑---L-N(G)-Nitro arginine methyl ester(L-NAME, 20mg/kg, i.p.),其餘步驟與第三組相同。另外,第五、六、七和八組,其處理和第一、二、三、四組相同,但是在福馬林或normal saline注射前及後5、15、30、45、60分鐘分別抽血測量NO的含量。結果顯示結果顯示電刺激坐骨神經和L-NAME 兩者都能減少Early phase的Licking和Biting,以及Lately phase的Licking 和biting。Formalin注射後5分鐘,它們血液中的NO 濃度的變化和注射Normal saline相似,但用L-NAME 前治療,則比Formalin有較低的NO濃度,而電刺激坐骨神經,則沒有和L-NAME 得到相似的結果。Formalin注射後30 分鐘和45分鐘,血液中的NO 變化比注射Normal saline高,而電刺激坐骨神經和L-NAME 前治療兩者都能減少NO 的濃度。我們的結論是電刺激坐骨神經減少Formalin 誘發Lately phase 的疼痛與NO有關。

    Our previous studies have known that electric stimulation can reduce formalin-induced pain, but its analgesic mechanism remains unclear. Nitric oxide (NO) is one of neurotransmitters, and it plays an important role in maintaining normal physiological function of human body. NO in the spine cord can regulate the morphine for pain relief, and in the mode of epilepsy or cerebral ischemia, the electroacupuncture (EA) can reduce the production of NO has been found, therefore, suggesting that have a closely relationship between NO and EA. The aim of the present study was to investigate the relationship between NO and electric stimulation of peripheral nerve. A total 48 male Sprague-Dawley (SD) rats were studied, they were divided into eight groups of each six rats as follows: 1) Group I: normal saline (50 ml) was injected subcutaneously into the left plantar surface of the rats; 2) Group II: formalin (5%, 50 ml) was injected subcutaneously into the left plantar surface of the rats; 3) Group III: 2Hz electric stimulation was applied to left sciatic nerve 15 min before and after formalin injection; 4) Group IV: L-N(G)-nitro arginine methyl ester (L-NAME, 20mg/kg, i.p.) was administration 15 min prior to formalin injection. In addition, the methods were the same as mentioned-above Group I, II, III, and IV in the Group VI, VII, VIII and IX, respectively, but the NO levels of peripheral blood were measured before, and 5 min, 15 min, 30 min, 45 min and 60 min after normal saline or formalin injection, respectively. The results indicated that both electric stimulation of sciatic nerve and L-NAME can reduce icking and biting in the early and lately phases in the formalin-treated rats. NO levels changes were similar between 5 min after normal saline-treated rats and formalin-treated rats. NO levels were lower in the L-NAME pretreatment group than formalin-treated group, but no similar changes were found in the electric stimulation of sciatic nerve group. NO levels were greater in the formalin-treated group than normal saline-treated group in the 30 min and 45 min after formalin administration, whereas that increase can be reduced by electric stimulation of sciatic nerve or by L-NAME pretreatment. Our conclusion is that they have a closely relationship between the analgesic effect of sciatic nerve electric stimulation reducing formalin-induced pain in the lately phase and NO.
