隨著醫療科技、技術之進步,醫療人員往往被認為具有極高之專業能力,人們對於醫療服務極為信賴,因此容易忽略醫療服務所帶來之傷害,再者醫療服務本身具有一定風險及不確定性,故病人在接受醫療服務之安全令人憂慮。醫院急診由於其獨特性,為醫院內最易發生醫療過失之部門,其中病人安全之重要性,相較醫院其他部門更為重要。故本研究欲統整國內外病人安全推行之情形,並了解台灣急診單位病人安全之努力與投入,以給予醫療院所及衛政機關作為未來努力及決策之建議。
本研究屬二級資料分析研究,資料庫資料單位為119家醫院急診單位。透過研究資料庫,本研究從領導、策略規劃、資訊與分析、人力資源、流程管理、及病患與家屬參與等角度,來了解醫院急診單位病患安全照護落實現況。共計八大構面、53個題項,以Likert 1分至5分方式量測之。本研究亦使用醫院急診單位組織編制特質之二級資料,包括醫院急診單位經營方式、主管及管理型態、全天候醫療服務、緊急醫療救護責任單位、急診專科醫師數、急診單位正式化程度、服務科別數、醫院評鑑等級及區域別等。以描述性統計、獨立樣本t檢定、變異數分析、簡單迴歸分析及複迴歸分析等統計方法來分析。
研究結果顯示,醫院急診單位病患安全照護落實程度的得分平均值,介於2.50分至2.10分之間。醫院急診單位為固定部門編制、設有急診專任主任、較多急診專科醫師數,以及醫學中心急診單位等等,與病患安全落實呈現正相關。
Abstract
As medical technology and skills progress, medical staff has been assumed to have highly professional skills and abilities. The public is greatly confident with the medical services; therefore, easily ignore the harms which medical service might bring about. Furthermore, medical services itself is provided with certain risks and uncertainties which in turns cause concerns on patient safety during medical serviced. Due to its uniqueness, emergency department is a unit with most medical failures in hospitals. Consequently, the importance of patient safety is more critical at the emergency department compared to the other departments in hospitals. The purpose of this study is to summarize the ongoing patient safety and to understand the efforts and inputs of patient safety at the emergency units of Taiwan, at the same time, provide beneficial suggestions to hospital administrators as future counsels.
This study was a second data analysis carrying on the data of 119 hospital emergency units. By analyzing the database, this study tried to understand implementation of patient safety of hospital emergency units, from the leadership, strategic planning, information and analysis, human resources, process management, and patient and family involvement perspectives. Eight dimensions with 53 items were included in this study, measured by Likert 1 to 5 points. Organizational characteristics, including business, leader and management type, 24-hour service, joining regional emergency networks, emergency physicians included, formalization, the number of medicine service provided, accreditation status and geography in the hospital emergency department were also included. Descriptive statistics, single sample t-test, analysis of variance, and simple and multiple regression analyses were performed in this study.
The average scores for the implementation of patient safety care of hospital emergency departments were ranging from 2.50 to 2.10. The hospital emergency departments characterized as departmentalization, with fixed-duty department managers and more certificated emergency physicians, and accreditated as medicine center level, were positively related to the implementation of patient safety care of hospital emergency department. |
