    Title: 整合無線感測網路技術於手術室排程系統之研究;Application of integrated wireless sensor network in operation room for surgical scheduling

    Abstract: 美國醫學會(Institute Of Medicine, IOM) 在2000年所發表的報告書”To Err Is Human”中指出,醫院每年約有44,000至98,000人因醫療疏失而導致死亡。手術相關疏失發生的頻率僅次於用藥相關疏失,係為最常導致病患死亡的原因之一。提昇手術室的病患安全可藉由有效的管理方式、手術程序的驗證、有效的人員溝通和資訊傳遞來改善。本研究採用無線射頻辨識技術(RFID)來強化手術室資訊系統,透過本研究所發展之系統得以正確辨識病患和醫護人員,有效進行手術流程追蹤以及藥、衛材的管理,預防醫療疏失。系統發展採用系統雛型法(System Prototyping)進行開發,包含控台、開刀房、恢復室、供應室等子系統,透過自動化流程進行資訊的蒐集作為手術室相關的人力資源架構改善之依據。系統在中部某區域醫院開刀房進行初步的臨床流程測試,結果顯示本系統能夠於開刀流程能夠有效地預防醫療疏失。採用科技接受模式(TAM)進行系統的認知有用性與認知易用性評估的結果發現71.43%的護理人員認為本系統是有用的,80%的護理人員認為本系統是容易使用的。一般而言,醫療機構於開刀房建置本研究所發展的開刀房管理(ORM)系統,可以提升病人及醫護人員辨識及手術流程管理,有效提升病人安全。

    In the United States, about 44,000-98,000 people died from medical errors each year. Surgery-related error, second to drug-related error, is one of the most commonly found errors resulting in premature deaths of patients. Improvement of the operation room (OR) environment through efficient checking and management of surgical procedures, identification of patients and healthcare providers, and information communication among healthcare professionals can highly elevate the patient safety. In this study, radio frequency identification (RFID) was used to strengthen the OR information system for efficient identification of patients and healthcare providers, tracking of critical surgery procedures, and control of medications and medical materials to prevent medical errors. A prototypic system which composes of console, operation room, recovery room, and supply room subsystems is presented. A preliminary clinical test was done at the surgical department of a district hospital in Taichung area. The system demonstrated usefulness in preventing medical errors under surgical setting. Finally, technology acceptance model (TAM) was applied to assess the perceived usefulness of the system and ease of use of cognitive assessment. The assessment shows that 71.43% of the nursing staffs perceive that the system is useful and 80% of them believe that the system is easy to use. In conclusion, the designed operation room management (ORM) system is efficient in elevating identification of patients and healthcare providers, management of surgical procedures, and communication among healthcare professionals, thereby increasing the patient safety.
