摘要: | 通過穴道雷射照射的光生物調節(photobiomodulation, PBM)被認為是疼痛控制的多模式管理之一。在之前的研究中,我們測定了低能量雷射針灸(LLLA)對術後疼痛的療效,並觀察了脊髓內的分子改變,在此研究中,我們設置這項研究以了解背根神經節(dorsal root ganglion, DRG)在同樣情況下的分子改變。此外,我們想確定高能量的雷射劑量是否能夠引起類似的鎮痛效果,並希望比較雷射照射傷口及穴道的止痛效果。
本研究使用足底切口(plantar incision, PI)疼痛模型,將62隻大鼠分成對照組,PI和PI + LA(雷射針灸)組。 PI + LA組進一步分為兩組,個別接受不同的雷射劑量,11.46J / cm2和343.95J / cm2,並分別命名為LLLA (L)組和LLLA (H)組。雷射針灸組中的所有大鼠皆相同地照射於右腿足三里上,照射範圍和持續時間也皆相同。我們發現兩種LA強度都能夠通過減少術後觸覺異常性疼痛來誘導鎮痛,然而,LLLA擁有更好的鎮痛效果。我們還發現雷射針灸比傷口照射產生更快的疼痛抑制作用。在DRG中,西方點墨法(Western blotting)揭示了絲裂原活化蛋白激?(MAPK)、iNOS和TNF-α的降低;然而,pERK的表現在劑量不同的低能量雷射組之間顯示出了差異性的變化。
Photobiomodulation (PBM) via laser irradiation at acupoint is suggested to be one of multimodal managements for pain control. Previously we had examined the efficacy of low-level laser acu-puncture (LLLA) on postoperative pain and observed the molecular alteration within the spinal cord, herein, we setup this study to understand the molecular alterations of dorsal root ganglion (DRG) under the same circumstances. In addition, we want to determine whether a high-level laser dosage would induce similar analgesic effect, and wish to compare the efficacy between irradiation on wound and acupoint.
By using a plantar incision (PI) pain model, 62 rats were sep-arated into groups of control, PI, and PI+LA (Laser Acupuncture). PI+LA groups were further divided into two groups to receive dif-ferent laser dosages, 11.46J/cm2 and 343.95J/cm2, which were named as LLLA (L) and LLLA (H). All the rats in the laser acu-pucture groups had the same laser point (acupoint ST36) at right hind leg, the same irradiated area, and the same duration. We found that both LA intensities were able to induce analgesia by reducing postoperative tactile allodynia at the paw, however, LLLA yielded a better analgesic effect. We also found laser acupuncture produced faster inhibitory effect than LLA irradiation at the wound. In DRG, western blotting revealed decreases in mitogen-activated protein kinases (MAPKs), iNOS and TNF-α level, however, diverse changes in the pERK level was discovered between different dosages of low level laser acupuncture. |
