摘要: | Background: Individuals with depression may experience considerable psychosocial adjustment, including occupational functioning, which is likely to affect employment status in a long run. Population-based study evaluating the long-term employment outcome in patients with depression is limited.
Objectives: We aimed to examine change of employment status among patients with depression, compared to controls without mental illness up to 12 years using national claim data of Taiwan.
Methods: A cohort of 3,673 patients with depression was identified from the National Health Insurance Research Database (NHIRD) of Taiwan between 2002 and 2008, compared to matched controls without mental illness. All participants were income earners when entered the analysis and were observed till December 31, 2013. The primary outcome was the time to change of employment status, determined as the change from income earner to non-income earner. Sociodemographic and clinical factors associated with risks of changing employment status were also explored.
Results: The adjusted hazard ratio of changing to non-income earner was 1.24 (95% CI, 1.10 – 1.40) in patients with depression over time as compared to controls. Multivariate logistic regressions revealed that younger age (18-25 years, aOR: 2.75, 1.96 – 3.84), lower payroll bracket (< NT$20,000, aOR: 7.28, 3.17 – 16.8), urbanity and geographical area were associated with risks of changing from income earners to non-income earners among patients with depression.
Conclusion: Patients with depression had a greater risk of changing employment status over time; however, most of them would remain employed despite of being underperformed, before and after the incidence. Future studies are warranted to improve treatment effectiveness and to support employees with depression reducing impacts of presenteeism in the workplace. |
