    Abstract: Background: Congenital hypothyroidism is the most common cause of preventable of mental retardation. Recently, transient congenital hypothyroidism becomes important part of congenital hypothyroidism. This study found out predictor to distinguish transient and permanent congenital hypothyroidism before trial off therapy to avoid adverse effect of treatment.
    Method: A retrospective cohort study was conducted at Children’s Hospital of China Medical University from 2003 to 2013. All subjects were diagnosed by newborn screening, treated with Levothyroxine and underwent trial off therapy at 3 years old.
    Result: The ratio between transient and permanent congenital hypothyroidism was 2.5:1. Levothyroxine dose at 6 months of age and 3 years old were strong predictors with optimal cut off point as follow 5.43 mcg/kg/day, 3.25mcg/kg/day.
    Conclusion: Transient congenital hypothyroidism was an important part of congenital hypothyroidism. Levothyroxine dose at 6 months of age and 3 years old can be predictors to distinguish TCH and PCH before trial off therapy. So re-diagnosed can be carry out earlier time between 2 and 3 years for patient who required low dose LT4.
