    Title: 寸口脈位壓力波訊息研究
    A Study on the Pulse Pressure Wave Information at Tsun-Kou

    Abstract: 根據《難經》,脈診是在寸口「單一位置」分輕重五段下壓;或同時在「三部」向下按壓「三候」。現代脈診研究在選擇量測位置時,則有許多種不同的方法!因此有必要先以實驗數據來回答:「寸口脈位壓力波在不同位置是否相同?」。
    本研究之第一部份在2011年7月至2012年10月於三軍總醫院進行「疾病組」之收案。方法為擬定三部九候的操作型定義,再使用PDS-2010 Skylark Pulse Analysis System,進行脈波量測;接著,運用SPSS 22.0與R軟體3.4.1版,以盒形圖、折線圖、與隨機區集多變量變異數分析,進行雙手三部九候之時域參數與頻域諧波的觀察與一致性檢定。第二部份於2018年6月,於小南門區民活動中心進行「健康組」之收案。
    「疾病組」的時域參數分析結果顯示:在同一部的不同候;與在「浮」與「中」候的不同部,其時域參數之一致性,達到統計顯著差異(p<0.05);「健康組」之結果亦同;而「疾病組」頻域諧波的分析結果呈現:個別的諧波,在同一部、不同候(除C4、C5);以及在同一候、不同部(除C8、C10),亦達到統計顯著差異(p<0.05)。
    由於在雙手三部九候呈現的血流動力學與諧波特徵並無一致,因此無法彼此替代。建議未來脈診研究,宜謹慎選擇量測部位,以確保分析資訊之完整性。
    Objectives: This study aimed to clarify whether the hemodynamic characteristics and the spectrum harmonics are identical in the “three positions and nine indicators” in the pulse diagnosis in traditional Chinese medicine (TCM).

    Methods: A total of 37 subjects in CAD group were recruited and measured for pulse pressure waves at 18 locations (9 per hand) with standardized operation protocol. Bar graphs, line graphs, and multivariate analysis of variance (MANOVA) were performed on the data collected in this experiment with a randomized block design using SPSS 22.0 and R 3.4.1 to examine the hemodynamic characteristics (i.e., h1、h2、h3、h4、h1/t、AIx、h3/h1、h4/h1) and spectrum harmonics (i.e., C0、C1、C2……C10) to confirm the consistency at 18 locations. A total of 20 subjects in healthy group were recruited to further confirm whether the hemodynamic characteristics are identical at 18 locations.

    Results: The results showed significant differences (p<0.05) among hemodynamic characteristics and spectrum harmonics at the 18 locations of “three positions and nine indicators” of both hands.

    Conclusions: Due to the differences of the hemodynamic characteristics and spectrum harmonics among the different positions and indicators, the study might provide a new opinion for future pulse diagnosis investigations to carefully consider and choose the measurement location to ensure the completeness of the pulse information.
