摘要: | 目的:膽固醇是阿茲海默氏症的危險因素之一,而Statin類藥品是目前效果最強且最廣泛使用的降膽固醇用藥。本研究目的為探討Statins類用藥與認知功能障礙、失智症及阿茲海默氏症發生之相關性。
方法:資料來源為2001至2010年「全民健康保險學術研究資料庫」之百萬承保抽樣歸人檔(LHID2005);利用Cox Proportional Hazard Model分析追蹤2001至2010年Statins用藥病患發生認知功能障礙(ICD-9:438.0, 438.1, 310.1)、失智症(290, 294.1)及阿茲海默氏症(331.0)之風險。
結果:2001年至2010年研究對象者共824,997 人,其中使用Statins藥品為14,491人佔1.76%。整體而言,使用Statins藥品者發生認知相關疾病風險為未使用者的1.37倍(95% C.I.:1.27-1.47);其中,年齡與發生認知相關疾病達顯著差異,年齡越高發病風險越高;男性罹病風險為女性的0.94倍(95% C.I.:0.91-0.96);有腦血管疾病、頭部外傷、中樞神經感染及腦部腫瘤病史者,較無病史者高。
結論:本研究結果顯示使用Statins類藥品者發生失智症之風險較高,可提供醫療相關單位用藥之參考。
Objective: Currently Statins are the strongest effect and were used the most widely for lowing cholesterol, and cholesterol is a risk factor of Alzheimer's disease. In this study, we decided to investigate the correlation analysis of Statins, cognitive impairment, dementia and Alzheimer’s disease.
Method: Based on Longitudinal Health Insurance Database (LHID2005) in National Health Research Institutes within 2001-2010, the patients who were diagnosed cognitive Impairment (ICD-9 code: 438.0, 438.1, 310.1), dementia (ICD-9 code: 290, 294.1), Alzheimer’s disease (ICD-9 code: 331.0) within 2001-2010 were selected as the research subjects. Cox Proportional Hazard Model was applied to analyze the Statins and cognitive-related disease risks.
Result: The research subjects are 824,997 people within 2001-2010. The number of Statins user is 14,491 (1.76%). Overall, the Statins users' risks of occurrimg cognitive-related diseases were 1.37 times of nonusers' (95% CI : 1.27-1.47); age and cognitive-related diseases had significant differences (p<0.001), Thou who were older had higher risks; Male’s risks of occurreding cognitive-related diseases were 0.94 times of female’s(95% CI : 0.91-0.96); Cerebrovascular disease, history of Traumatic Brain Injury, Central Nervous System Infections and Neoplasm of brain were higher than those who didn’t have.
Conclusion: The result indicated that those who used Statins can increase the risk of occurring dementia. This result can provide a information for the medical treatment units. |
