    摘要: 醫院因應環境改變及響應顧客就醫滿意之潮流,希望藉由推行服務最優化及強調人性化照護於臨床,提供高品質照護。本研究想從醫病溝通的角度,探討執行醫護家庭會議之溝通模式的醫護人員與家屬之間關係的建立,進而影響到其醫病關係品質。本研究的目的有四:一、瞭解病患家屬對醫護家庭會議之溝通模式的醫病溝通知覺、醫病關係品質各變相的描述性統計。二、探討影響醫病溝通知覺與醫病關係品質的相關人口學變項因素。三、探究醫護家庭會議之溝通模式的有無對醫病溝通知覺及醫病關係品質的差異與影響。四、分析醫病溝通知覺在醫護家庭會議之溝通模式與醫病關係品質的關係間所扮演的中介角色。以問卷調查方式,採便利取樣方式選取中部設置有呼吸照護中心的某四家醫院進行取樣發放問卷,以入住呼吸照護中心之所有病患家屬為研究對象,由訪員進行實地的問卷調查,且該主要決策者能以國台語溝通。結果顯示,醫護人員採取醫護家庭會議之溝通模式對醫病溝通知覺及醫病關係品質有顯著的影響;病患家屬的溝通知覺對醫病關係品質有顯著的影響。這兩項構念亦會透過醫護家庭會議之溝通模式,進而正向影響醫病溝通知覺及醫病關係品質。
    To cope with environmental changes and the trend of patient satisfaction, hospitals would promote service optimization and emphasize clinically humanized healthcare to enhance the high-quality healthcare. From the aspect of physician-patient communication, this study tends to discuss the effects of the relations between medical personnel and family members in the practice of Communication Model for Family Healthcare Conference on Physician-Patient Relationship Quality. The research purposes contain 1.to understand the descriptive statistics of Perceived Physician-Patient Communication and Physician-Patient Relationship Quality in Communication Model for Family Healthcare Conference, 2.to explore the effects of demographic variables on Perceived Physician-Patient Communication and Physician-Patient Relationship Quality, 3.to discuss the effects of Communication Model for Family Healthcare Conference on Perceived Physician-Patient Communication and Physician-Patient Relationship Quality, and 4.to analyze the mediating effects of Perceived Physician-Patient Communication on the relations between Communication Model for Family Healthcare Conference and Physician-Patient Relationship Quality. With questionnaire survey, convenient sampling is utilized for selecting four hospitals with Respiratory Care Center in central Taiwan. The patients in the respiratory care centers and the family are distributed the questionnaire, and the interviewers are able to communicate with both Mandarin and Taiwanese. The results show the significant effects of medical personnel practicing Communication Model for Family Healthcare Conference on Perceived Physician-Patient Communication and Physician-Patient Relationship Quality as well as the remarkable influence of patients and the family’s Perceived Communication on Physician-Patient Relationship Quality. Such two dimensions would positively affect Perceived Physician-Patient Communication and Physician-Patient Relationship Quality through Communication Model for Family Healthcare Conference.
