摘要:	本研究目的在探討中醫穴位按摩法運用於極低出生體重早產兒之效果。採準實驗法。第一年請專家根據作者過去研究所建立的經絡與穴位按摩模式加以簡化。並進行簡化按摩模式之先驅研究（pilot study），預收8 位。第二年及第三年採準實驗研究法，收集實驗與對照組個案驗證經絡與穴位按摩之簡化模式。研究對象來自某準醫學中心病嬰室之極低出生體重早產兒（體重小於1500 g），以立意取樣方式，選擇符合研究條件之個案80 位。依研究對象出生體重隨機分派為兩組，對照組個案僅提供病嬰室常規護理，不介入按摩。穴位按摩組先觀察1 天，再介入簡化按摩15 天，按摩結束後，繼續觀察並記錄至出院。按摩組每天按摩三次，約於10AM、1PM 及4PM，每次15 分鐘，使用簡化的經絡與穴位按摩模式。採自擬之結構式問卷收集資料，包括1.早產兒的基本資料、住院天數等2.按摩前中後監測體溫、脈搏、呼吸、氧飽和濃度3.下午按摩前後測量早產兒活動度、活動睡眠狀態及行為窘迫現象各10 分鐘4.評估早產兒發展狀態 5.每天測量兩組早產兒的體重，並記錄其餵食量及排便量，一直到個案出院6.追蹤兩組個案出生滿三個月的體重。按摩信效度方面，收案前進行研究員訓練，並儘量由同一人執行按摩。以SPSS＋ 12.0 版進行資料分析，採用次數分配、百分比、平均值、標準差、χ2-test、t-test、paired t-test、regression 及Mixed model 等統計方法。 The purpose of this study is to investigate the effects of acupressure application in very low birth weight premature. The first year, experts panel will establish a simplified model of acupressure of premature, and 8 participants will conduct a pilot study. A quasi-experimental study is designed for the second and the third year. Eighty premature babies will be recruited by purposed sampling, according to the criteria of very low birth weight (<1500gm) from a sick baby room in a medical center. The control group only receive routine care in sick baby room unite. The babies in experimental group will be observed for one day, and then intervene the simplified acupressure three times a day for 15 days, and continuously recoding the observation and records until discharge. The intervention includes acupressure for 15 minutes each at 10Am, 1Pm and 4Pm. The data will be collected by a self-developed structured questionnaire, including: 1) demographic data of premature. 2) The oxide saturation level, body temperature, and pulse and respiration rate before, during and after acupressure. 3) A 10 minutes observe before, during and after acupressure about premature in activities, sleep status, and behavioral distress. 4) Developmental status in both groups. 5) Records in feeding amount, elimination amount, and body weight every day until discharge, both groups. 6) The body weight of third month in both groups. A trained researcher wills performance in acupressure to ensure the consistency and accuracy of intervention. The data analysis will be conducted by SPSS 12.0, using frequency, percentage, means, standard deviation, 2-test, t-test, paired t, regression, mixed model for the proposal of the study.
