    Title: 電針刺激對糖尿病大鼠神經再生之影響
    The effects of electroacupuncture on nerve regeneration in diabetic rats.

    Abstract: 我們討論的是在經過周邊神經截斷手術之後,以1 mA及2 Hz的經皮電針刺激環跳穴及陽陵泉穴之後是否可以促進糖尿病大鼠軸突的再生和功能性的回復。四組成鼠(W組:是控制組沒有DM[沒有誘發成糖尿病鼠];X組:是DM控制組; Y組:是DM+針灸;Z組:是DM+電針)之坐骨神經被截斷後,其兩斷端以矽膠導管進行修補使之間距為10 mm。Z組的大鼠,三個星期內每隔一天其穴位上被施加15分鐘的電針刺激;X組與Y組則未被施加電針刺激。手術後4星期,免疫組織學染色呈現Z組相較Y組之動物,與創傷同側之脊髓灰質背角之降鈣素基因相關胜肽及遠側斷端神經中之巨噬細胞數有明顯增加的現象。在X組與Z組之大鼠,其電生理的結果相對於Y組顯示出較高層級的神經重新支配,以及顯著較短的潛伏期與較快速的神經傳導速度;組織學的評估亦呈現出X組與Z組之大鼠相對於Y組有相對較高的髓鞘軸突密度與神經內膜面積。由此可知,電針刺激可以改善糖尿病動物周邊神經嚴重受損後的復原率,而電針刺激應可作為治療糖尿病神經損傷的另類療法。

    We discuss if electroacupuncture (EA) at Huantiao (GB30) and Yanglingquan (GB34) at 1 mA and 2 Hz after peripheral nerve transection could enhance axonal regeneration and functional recovery in diabetic animals. Four groups of adult rats (group W: normal rats; groups X ,Y and Z : streptozotocin diabetic rats) were subjected to sciatic nerve section followed by repair using silicone rubber conduits across a 10-mm gap. Rats in group Z received EA for 15 minutes every other day for 3 weeks. The groups X and Y received no EA. At 4 weeks after surgery in group Z, immunohistochemical staining showed that lamina I-II regions in the dorsal horn ipsilateral to the injury were significantly calcitonin gene-related peptide-immunolabeled and a significantly higher number of macrophages were recruited in the distal sciatic nerve compared with Y. In groups X and Z, electrophysiological results showed higher levels of reinnervation with significantly shorter latencies and faster nerve conductive velocities, and the histological evaluations showed relatively larger mean values of myelinated axon densities and endoneurial areas compared with Y. Thus, the EA may improve the recovery of a severe peripheral nerve injury in diabetic animals, which could be considered as a supplementary treatment in diabetic neurotrauma.
