摘要:	本研究針對台灣地區具有技術士證照之電焊工為族群，與勞、健保等相關資料庫進行串連、分析及統計工作。研究方法主要分為四部份：（1）電焊技術士世代資料庫建立（2）健康相關指標分析：以年齡、傷病分類、事故原因、媒介物、受傷部位、職業病成因、及殘廢項目等分類統計，進一步分析電焊工職業傷害及疾病的狀況（3）計算罹患各項癌症之標準化發生率比（SIRs）、罹患各項癌症標準化死亡率比（SMRs）、罹患各項系統器官疾病之標準化疾病率比（SMRs）、罹患人體各系統器官之所有疾病之盛行率（prevalence）（4）開發資料庫軟體以整合所有資料表於單一軟體，並提供住院盛行率統計功能，本研究重要結果如下：一、整合電焊技術士世代資料庫檔，開發專用單機化資料庫程式。二、在16,512位電焊技術士中，以一般手工電焊證照的人數最多佔84.66％，職業危害性又以一般手工電焊較高，因此一般手工電焊證照技術士更應為安全衛生監控的重點族群。三、本研究電焊技術士癌症發生率前三名為肝癌、鼻咽癌、口腔癌，以全國一般民眾為對照組進行標準化後之SIRs則以造血系統及網內皮系統癌有較明顯的突出，與過去文獻相符。四、以證照年來推斷電焊技術士之工作年資恐有低估情勢，但部分癌症之趨勢上揚轉折點仍具有參考價值，秉持預防優於疾病發生仍可作為後續安全衛生預防參考。五、電焊技術士得到癌症之標準化死亡率有超過一半以上的值是大於一般民眾的（52.63%），與過去研究電焊工得到癌症的死亡率有增加的趨勢相符。六、疾病標準化發生率上電焊技術士以呼吸系統疾病、神經系統及感覺器官之疾病、眼及其附屬器官之疾患高於一般民眾且與過去文獻相符。七、職業傷病以被夾被捲及墜落滾落佔最大宗，特別注意感電事故為電焊技術士死亡給付排名第四位。八、電焊技術士65歲以上缺血性心臟病、肺癌致死比例高，與過去研究相符合，加上高年齡同時也伴隨高電焊職業暴露年齡，這點值得注意。九、電焊技術士行業別，依證照別分，六種證照大部分都以製造業及營造業的行業佔較高的比例，顯示出電焊技術士大多仍以從事電焊相關行業為主。本研究對象族群囊括台灣地區全體電焊技術士，經由本計畫之研究結果，得以了解其職業暴露對健康之效應及職業傷亡發生情形，以提供未來預防職業傷病策略之參考。; The purpose of this is to analyze potential health outcomes among certified welder technicians of Taiwan via analyzing the health-related factors, indicating the database of Labor Insurance and National Health Insurance. The methodologies of this study are mainly divided into four portions: (1)To establish generational database of the welder technicians (2)To analyze the health-related factors of the welder technicians, including age, category of injury illness, reasons of the incident, injured location, cause of the occupational disease, and items of disabled, etc., with their occupational injury and sickness condition (3)To determinate the standardized incidence ratios of the welder in suffering various cancer (SIRs), the standardized mortality ratios of the welder in suffering various cancer(SMRs), the standardized morbidity ratios of the welder in suffering various systematic disease (SMRs) and the prevalence of the welder in suffering various systematic disease (4)To design a computer software for integrating multiple databases and providing prevalence of statistical fuction. Major findings of this study are summarized as follow: 1.Integrating multiple databases of welder technicians to develop professional software for executing in a single computer. 2.The most number of 16,512 welder technicians is general manual welder technicians (84.66%) with greater job harmfulness. As a result, they should be majored to control for the safety hygiene. 3.The top three cancer incidences are liver cancer, nasopharyngeal carcinoma and oral cavity cancer. On the other hand, Hemutopoiesis system and network leather system are higher than standard of general population which is consistent with past studies. 4.It might be underrated to estimate working seniority by license year. However, the turning points of some increasing cancer cases still can be the reference to health prevention. 5.According to the past studies, more than half of the standardized cancer death rates of welder technicians are greater than general population (52.63%). 6.The standardized incidence rates of welder technicians in respiratory system, nervous system, sense organ and eye are higher than general population that is in conformity with the past studies. 7.The most number of welder techniciasns occupational injury cause by jam and fall. And especially taking notice of the electric shock accident which is the fourth cause of death. 8.Conformity with the past studies, the death proportion is high in welder technicians who are over age 65 by ischemic heart disease and lung cancer. Moreover, it should be noticed that the elder welder technicians usually have longer job seniority. 9.Welder technicians are divided by different licenses. A high percentage of these licenses are related with manufacturing and building industries. It shows they are engaged in relation jobs.
