    题名: To Induce a Better Controlled Barbiturate Coma by the Using of Bispectral EEG Monitor

    摘要: Barbiturates are commonly used to suppress electroencephalogram (EEG) burst and induce coma for the treatment of refractory generalized status epilepticus. Due to the inconvenient continuous bedside monitoring with a full polysomnogram EEG, the dosage of barbiturate cannot be accurately calculated and serious side effects often occurred. These factors deter clinicians from instituting suitable EEG burst suppression treatment. The bispectral index (BIS), by time dormain analysis of EEG power spectrum and bispectrum, is one of the most convenient and reliable alternative methods. Here we report a case of a 63- year-old patient who was suffering from status epilepticus, and was treated with barbiturate coma. Under continuous BIS monitoring, therapeutic barbiturate coma for refractory generalized status epilepticus could be safely and effective delivered for long term administeration. Promptly treat the refractory seizures using an anticonvulsant regimen that could be tapered without seizure recurrence and allow the patient to recover from the effects of therapy as rapidly as possible was reached by BIS application.?
