    摘要: 自民國71 (1982)年起癌症即為國人十大死亡的首因,且每年罹患癌症的病人遞增。根據衛生署的統計資料,民國91年初次診斷為癌症的病人有6萬人左右,預計 2020年將上升至10萬人。紫杉醇為自1990年以來最重要的抗癌藥物之一。除已被批准的適應症-卵巢癌、乳癌、肺癌外,它對頭頸癌、鼻咽癌、食道癌、胃癌、膀胱癌等亦具療效。化療藥物紫杉醇針劑在台灣的一年用量可達6-7萬支,約值2-3億的費用。癌症病人於接受紫杉醇化療後除了有骨髓抑制的不良反應外,亦伴隨產生噁心、嘔吐、口腔潰瘍、食欲不振、身體衰弱等副作用,也有病人因身心的負擔而造成憂鬱症。很多病人因此尋求中草藥或健康食品以緩解身體的不適、或補血益氣,以提昇免疫力和體力。常用的中草藥或食物與西藥的交互作用的現象日漸被國內外重視,最著名的2個例子為葡萄柚中的黃酮類 (flavonoids)和抗高血壓、治療癲癇等西藥之交互作用;及改善憂鬱症的聖約翰草(St. John's wort)的萃取物和抗癌藥物喜樹鹼類(camptothecins)的之交互作用。中草藥和西藥併用也可能帶來好處,例如因中草藥、抗癌藥物併用,而提高抗癌藥物效果,或減半抗癌藥物使用劑量。雖然目前看到僅少數的文獻,但可成為新的硏究方向。 本計畫之目的擬以大鼠和裸鼠(xenograft model)的動物模式評估紫杉醇和黃芩、甘草、丹參、六味地黃丸、補中益氣湯等的PK/PD交互作用,以提供中草藥服用安全的資料,也擬從中發掘一些單味藥或方劑或其中一些成分,可和抗癌西藥併用以增加其療效,減少抗癌西藥的用量,除了可以節省醫療費用外,也可能降低癌症病人服用抗癌西藥的不適症狀,改善其生活品質,作為未來提供衛生主管機構對於中西藥併用於特殊疾病,如癌症用藥之參考資訊。

    Cancer has become the leading cause of death in Taiwan since 1982 and its incidence rate increases yearly. There were about 60 thousands Taiwanese people who were diagnosed cancer in 2002 and the numbers will be close to 100 thousands in 2020 according to the statistics report of Department of Health. Paclitaxel is one of the most important anti-cancer drugs since 1990. It is effective for breast, ovarian, lung, head and neck, nasopharyngeal, esophageal, stomach, and bladder cancers. It is widely used to treat cancer patients and the consumption is about 60-70 thousands vials a year, market value about 200-300 million NT dollars. The toxicities caused by paclitaxel treatment include myelo-suppression, nausea, vomiting, stomatitis, loss appetite, and anesthesia. Cancer patients sometimes had depression due to the physical and psychological burdens resulted from the disease. Many cancer patients seek for alternative medications or health foods to ease the side effects caused by paclitaxel treatments, to recover from the physical weakness, or to boost their immune system. Food-drug or herb-drug interactions have raised attention of patients, health department or researchers world-wide recently. The most famous examples are grapefruit juice and anti-hypertension drug interaction and St. John?HH?HHs wort and anti-cancer drug irrinotecan. However, herb-drug interaction can be beneficial because some herbs may potentiate the anti-tumor activity of anti-cancer drug or decrease the required dosage for effective treatment of anti-cancer drug. The purpose of the present study is to evaluate the PK/PD interactions of paclitaxel and traditional Chinese Medicine, such as Huang qui, Dan Shen, etc., using rat and human tumor xenografted nude mice as models; and to provide these safety and efficacy information to the Health Departments for future references. In addition, to explore the possibilities of finding active ingredients from these traditional Chinese Medicine to potentiate the effectiveness of anticancer drugs, such that to save the chemotherapy cost, reduce the side effects caused by anticancer drugs, and improve the quality of life of cancer patients
