    摘要: 我們已經證實右美沙芬 (dextromethorphan) 及其代謝產物 (metabolities) 具有大鼠坐骨神經阻斷 (sciatic nerve blockade)、浸潤性皮膚止痛 (infiltrative cutaneous analgesia)與脊髓麻醉 (spinal anesthesia) 效用,因此它們是局部麻醉劑 (local anesthetics)。雖然臨床上使用局部麻醉劑來處理急性與慢性疼痛已經超過一百年了,卻仍然缺乏長效的局部麻醉藥物或適當劑型以供臨床來使用。因此本研究目的將著重於右美沙芬及其代謝產物之藥物相互作用 (interaction) 與長效之局部麻醉效用與神經病理性疼痛 (neuropathic pain) 效用探討,實驗分成三年:第一年研究、為加入藥物 (例如epinephrine或clonidine等) 與右美沙芬、右啡烷 (dextrorphan) 或甲氧基嗎啡鹼 (3-methoxymorphinan) 合併後給予並分別測試藥物相互作用或延長浸潤性皮膚止痛、坐骨神經阻斷或脊髓麻醉之效用。第二年研究、以二十五種長效緩釋局部麻醉劑型製備,並測試這些藥物之坐骨神經阻斷與皮膚局部止痛之效果。第三年研究、以十五種長效緩釋藥物測試這些藥物對神經病理性疼痛止痛效果。

    We had proved that dextromethorphan and its metabolites had sciatic nerve blockade, infiltrative cutaneous analgesia, and spinal anesthesia in rats. All of them are local anesthetics. Local anesthetics have been used in acute and chronic pain management for 100 years, but long-acting local anesthetics or suitable formulations are still not enough for clinical therapies. Therefore, our study will emphasize the interaction of drugs and long-acting effects of dextromethorphan and its metabolites on local anesthesia and neuropathic pain. This project will be carried out for three years. In the first year, combination of drugs (i.e. epinephrine or clonidine) with dextromethorphan, dextrorphan or 3-methoxymorphinan on the interaction or prolonging of the effect of infiltrative cutaneous analgesia, sciatic nerve blockade, and spinal anesthesia will also be determinated. In the second year, twenty-five oil dosage forms of local anesthetics will be formulated, and these formulations will be evaluated on infiltrative cutaneous analgesia and sciatic nerve blockade. In the last year, fifteen oil formulations will be evaluated on neuropathic pain.
