    题名: 癌症病人與病友團體及醫療機構間之供給和需求研究

    摘要: 由國內外文獻均顯示,支持性服務對癌症病人之身心支持有助益,進而提升病患生活品質。所以研究主要目的包括:1.探討癌症病人對病友團體/醫療機構服務之需求。2.探討病友團體/醫療機構對癌症病人服務之供給。3.癌症病人對病友團體/醫療機構服務之需求與病友團體/機構對服務供給之比較分析。最後4.依據文獻探討、供需現況分析結果及現行政策,提出建議。本研究以中部地區癌症病患及醫療機構/病友團體為對象,依立意取樣方式進行口腔癌及結直腸癌病患支持性服務需求的深度訪談調查,研究依研究程序分別各完成 15位個案(共30位個案)之深度訪問;而醫療機構及病友團體部份,則共進行8場次的焦點團體,以了解醫療機構及民間團體所提供的支持性服務內容。本研究結論,1.結直腸癌病患服務需求,以療程溝通、醫療諮詢、關懷訪視及支持團體服務,需求迫切性最高;2.民間團體在醫療、衛教、營養諮詢、病友關懷訪視及支持團體提供服務成效顯著;3.口腔癌病患主要服務需求,以療程解說溝通服務、營養衛教諮詢、營養衛教課程及經濟救助,需求最高;4.高死亡率及復發的特性是罹癌病患心理上的主要壓力源;5.抗癌療程中病患需要身、心、社會、靈的支持力量;6.醫療體系癌症服務偏重醫療層面,病患心理及社會層面服務相對缺乏:本研究建議如下1.面對社會對於口腔癌病友團體的標籤壓力,以更大範圍的頭頸喉癌病友團體的形式推動涵蓋性病友團體;2.運用醫療機構癌症防治中心個案管理及資源整合的功能協助癌症病人;3.以醫療機構為主體推動模式;4.以目前病友團體為主的推動模式;5.舉辦單元式病友團體活動累積經驗及人力資源;6.異業結盟的方式來推動。

    In the domestic and foreign documents articles reviewed it was shown that support service that had supplying from support group and medical institutions could be helpful both mental and physical to the cancer patient and improving quality of life at the same time. The main purpose of this study included: 1.to pursuit cancer patient demand services which provide from support group and medical institutions 2.on research support services supply on cancer patient from support group and medical institutions.3.to compare and analyze the demand services for cancer patient and support services supplies which provide from support group and medical institutions.4.for providing suggestions based on the documents survey, supply demand situation and current policy. In this study, selected through purposive sampling to realize patient' service demand and conducted with in-depth interview survey that proceed through oral cancer, colorectal cancer patient and support group, medical institutions that we had accomplished 15 subjects each(total 30 cases)where are covered middle regions of Taiwan; in order to realize the support service which provide from medical institutions and social groups, we had took eight focus group completed during the research procedure . The conclusion of the study: 1. The services of colorectal cancer patient demand " therapy communication"" medical consultation" concern visit" support group" which can provide fine medical resource and more support system to help; 2.Better patient prognosis have validness improving with medical, health education, nutrition consultation and support group concern visit which provide services from civil support group; 3. The services for oral cancer patient most demands are " therapy explain and communication service" nutrition consultation" economical aid", while those support services that provide from medical institutions still exist a large demand gap at present day; 4. Caner patient have psychical pressure for high mortality and recurrence shadow and consider that cancer is a rather complicated and difficult disease; 5. Support resource from physical-mental spirit and social support are necessary during medical care treatment period; 6. Medical institutions that focus on medical orientation management, on the contrary are lacking psych-social supports. In this study we suggested: 1.Due to oral cancer patient are facing society label pressure, a comprehensive support group which include head-and-neck and throat cancer patient may lead to further advance help; 2.Using function of case management and resource integration to assist cancer patient with medical institution cancer prevention center; 3. Through subjectivity principal to encourage medical institution with promote patient service mode; 4. Further advance primary service would come from those support groups currently for their highlight assistance to cancer patient; 5. On the support group improvement, apply singularize support group activities to aggregate experience and human resources; 6.Providing service resources enhance medical institutions assistance with collaboration integrated method.
