    摘要: 台灣地區衛生所在20世紀擔負了國家公共衛生行政與地方基層醫療的工作,舉凡預防注射、防疫、保健、群醫醫療等工作,甚至是偏遠地區的衛生保健工作。而在21世紀初,世界衛生組織(WHO)的「健康城市」及「健康社區」概念,以及民眾一生對健康保健的需求等等,使得國家的衛生醫療與國家整體社區經濟文化價值開始作了相當的整合,包括社區健康營造的落實、健康促進的規劃等等。因此,現階段的衛生機構高階主管的責任與工作角色相對上比過往來得吃重且複雜。衛生署於民國89年訂定「衛生所員額設置參考基準」,建議衛生所主任由醫事人員兼任,因此目前國內衛生所主任的專業背景包括醫師、非醫師以及具公衛背景之醫師等等;然而截至目前為止,尚無研究針對不同專業背景之領導者,其對於該組織運作與其領導效能提出具體的評估。因此本研究的目的在於:1) 了解台灣衛生所主任的專業背景分佈;2) 了解台灣衛生所之組織策略、內部運作與組織效能;以及3) 了解台灣衛生所主任專業背景差異對衛生所之策略、內部運作與效能之影響。本研究為一年期專題研究計畫,以台灣346家衛生所(排除台北市及高雄市衛生局所轄之衛生所)為研究分析單位,所得資料將從兩方面進行分析,即針對衛生所主任專業角色對其組織運作之影響的理論模型進行橫斷性資料分析,以及針對衛生所組織效能進行揉合資料(panel data)分析。在橫斷性資料分析方面,將結合敘述性統計(descriptive analysis)及結構方程模式(structural equation modeling: SEM)方式來探討衛生所的運作效能,即了解台灣衛生所主任專業背景差異對衛生所組織運作的影響,包括組織策略選擇、組織內部運作以及組織效能一整合組織運作方式與其效能間的相關性;而在揉合資料(panel data)分析方面,擬收集台灣所有衛生所1990年至2004年共15年的資料作為分析依據,來了解衛生所的組織變項對衛生所效能的影響。

    Health stations in Taiwan have been played an important role in the implementation of Taiwan health policies and primary health care, including disease prevention, immunization, health promotion, group medical care, cares in rural area, and so on, in the 20th century. In the 21st century, the concepts of "Health City" and "Health Community" proposed by WHO and emphasized by the people, further impose more and more tasks and responsibilities on the national health stations' leaders. In 2000, Taiwan National Health Department announced the qualification of profession education backgrounds to be a CEO in the health stations, including the education backgrounds of physicians, non-physician, and physicians with public health profession education. However, little studies have ever made efforts to understand the effect of the various profession education backgrounds of the health station leaders in Taiwan. Therefore, this study is aimed to examine the effect of the leadership on an organization's design and performance in the Taiwan Health Stations. The specified research questions include: 1) to understand the CEO professional education backgrounds in the individual Health Stations in Taiwan; 2) to examine the strategies, organization design, and performance for the individual Health Stations in Taiwan; and 3) to examine the effect of the professional education backgrounds of the CEO in a health station on a health station's strategies, internal process, and performance in Taiwan. Using individual health stations as unit of analysis, 346 health stations in Taiwan excluded Taipei and Kaoshung Cities, will be analyzed. Cross-sectional and panel data will be collected in this study. The relationship among CEO professional education background, organizational internal process, and performance will be examined for the individual health stations in Taiwan through cross-sectional data and LISREL statistical technique. The panel data of individual health stations' performance in the period of 1990-2004 will be collected and to examine the effect of organization characteristics on its performances.
