摘要:	古柯鹼濫用所引起的全身性強直陣攣發作（Generalized tonic-clonic seizures）和癲癇重積狀態（Status epilepticus）等神經系統後遺症已得到充分的證實。癲癇發作（Seizures）或許是因為偶然地古柯鹼服藥過量所誘發，而且使用相對較低的消遣劑量也可能發生。古柯鹼濫用導致猝死的原因包括心律不整、癲癇重積狀態和腦溢血。對人類及實驗動物來說古柯鹼致死的主要決定因素被認為和癲癇發作有關。針灸被中醫使用已長達2500年，近來愈來愈受到西方國家的歡迎與支持，不少臨床前與臨床數據證明針灸會造成多重生理反應。本研究探討以電針刺激小鼠大椎及百會穴位，是否可以用來治療古柯鹼誘發的死亡或癲癇。本研究將週齡雄性小鼠隨機分配為八組，分別為控制組、電針調變式（Modulation，MOD）2Hz、50Hz，持續式Constant， CST） 2Hz、50Hz、100Hz，單純針刺及非治療穴位組（Sham）。探討電針是否可以降低鹽酸古柯鹼腹腔注射誘發小鼠按Itzhak 5級觀察量表（5-stage observation scale）癲癇發作的程度、痙攣（Convulsion）發生以及死亡率。將電極分接在百會穴、大椎穴，動物採取非麻醉方式，使用電刺激器（Trio 300，Ito Co.，Japan），電流強度1mA，於腹腔注射古柯鹼前施行電針刺激。我們的調查發現，在電針“大椎”（GV14）和“百會”（GV20）穴位15分鐘以MOD50Hz的電針刺激能顯著降低古柯鹼誘發小鼠在單一劑量（75 mg/kg）癲癇發作(Itzhak 5級觀察量表)的程度且MOD50Hz、CST50Hz和CST2Hz的刺激能降低痙攣的機率。MOD50Hz也能夠明顯降低古柯鹼（60 mg/kg i.p）重複投予引起痙攣的機率。電針此二穴以MOD2Hz、CST2Hz和MOD50Hz的刺激甚至也具有降低古柯鹼（125 mg/kg）引起的死亡率，實驗結果顯示2Hz電針刺激具有最佳的效果。然而電針天宗（SI 11）穴位15分鐘（左右兩側）卻無此效。此外我們也以Procaine（400 mg/kg）腹腔注射誘發小鼠死亡，發現電針刺激不降低Procaine引起的死亡率。 Generalized tonic-clonic seizures and status epilepticus are well documented neurologic sequelae of cocaine abuse. Seizures may be induced by cocaine after an accidental overdose, as well as after the recreational use of relatively low doses of cocaine. Mechanism of sudden death in cocaine abuse include cardiac arrhythmia,status epilepticus and intracerebral hemorrhage. Seizures are considered a major determinant of cocaine-related lethality in humans and animals. Acupuncture has been used in traditional Chinese medicine for over 2,500 years. It has recently started to gain popular support and nowadays, it is increasingly practiced in Western countries.Weighing 28-35g was used in this study. The mice were randomly divided into eight groups、a control group、a sham group、a needles just inserted acupoints group、the constant waveform of electroacupuncture (CST EA, 2 Hz, 50Hz, 100Hz groups and the modulating waveform of electroacupuncture (MOD EA ,2Hz、50Hz) groups severally.The mice in EA group were then given EA on GV14 and GV20 bilaterally, fifteen mins at a time, without any anesthesia.Electrical stimulation was provided by a pulse stimulator (Trio 300, Ito Co., Japan), which produced a biphasic square wave with the following characteristics: 2、50 or 100 Hz frequency, 1mA duration pulses. Our investigations found that MOD50Hz EA at the “Dazhui” (GV14；大椎) and “Baihui” (GV20; 百會) acupoints for 15 mins significantly reduced single-dose (75mg/kg i.p) cocaine-induced Itzhak’s 5-stage observation scale of seizures and convulsions, repeated-dose (60mg/kg i.p)cocaine-induced convulsions in mice. Moreover EA at GV14 and GV20 also reduced the numbers of mice that succumbed to cocaine-induced (125mg/kg i.p) death and CST2Hz EA got the best affects. However, EA for 15 mins at the “Tianzong” (SI 11; 天宗) acupoint (left and right sides) had no such effects.In addition, EA had no such protection of Procaine (400mg/kg i.p.) -induced lethality in mice.
